A new way of caring for you in an emergency
In an emergency, or if your doctors surgery is closed, you might need care from a doctor or nurse who doesn't know you.
A summary Care Record is an electronic record that can help give you safer, faster care by giving healthcare staff instant access to a summary of important information such as allergies and medicines, reducing the risk of mistakes being made when caring for you in an emergency.
Your Summary Care Record will include your name, address, date of birth and your unique NHS number to help identify you correctly.
Healthcare staff will need to be directly involved with caring for you in order to see your record and will need to have an NHS smartcard with a chip and passcode and have their details recorded. They can only view information they need to do their job.
You can choose to opt out of this service by filling in an Opt-Out Form optout.pdf and returning it to the Health Centre Reception Desk.
